Personal information requested and collected by this business via our Contact Form is as follows: Full Name, Phone Number, Email Address, Reason For Appointment. Any and all personal information collected on this site is inputted by the user through our Contact Form and is submitted and stored in WordPress. Once the user has submitted their information via the website Contact Form, a member of our staff will access this information via our business’s WordPress account to reach out to the user directly via a secure line. Any and all discussions after this point are done via secure, HIPPA Compliant phone lines or email chains and solely for the purpose of educating the user on potential treatments and scheduling the user for an appointment. Once a staff member has contacted the user via a secure line and the user has agreed to schedule an appointment, the staff member will begin the Patient Intake process via Mangomint. Mangomint is a HIPPA Compliant platform for scheduling and payments used by
our business.The staff member will input the following information from the user into Mangomint: Legal First & Last Name, Phone Number, Email Address, Credit Card Information. The credit card information may be offered to the staff member by the user on a secure line via a text message or via a verbal phone conversation. This credit card information will be safely stored in Mangomint and will never be used unless the patient has a Late Cancellation or a No-Show. The staff member will then schedule an appointment for the user on a day & time that works for the user, and send the user a Form Link via Mangomint. The HIPPA Compliant Form Link will be sent to the user’s phone number and email. The forms in the Form Link request information on the user’s current ailments along with their medical history. The forms also ask for consents from the user, inform them of their rights under HIPPA, disclose our cancellation policies and our financial policies.The staff member will also input the following information into Charm EHR, our business’s medical records and medical charting system: Legal First & Last Name, Date Of Birth, Gender, Phone Number, Email Address.
